Please Read This Page Before Downloading the Form
The following document includes all of the necessary INFORMATION FORMS and SIGNATURE FORMS that need to be downloaded, typed, then printed, and signed or printed, then hand written, and signed by the Sponsoring Rotary Club. Even the forms that need no signatures should be read extremely carefully or the respected club will be ill prepared for camp.
Rotary Clubs Please Follow These Instructions On How To
Download, Fill Out and Print the Forms
1. Click on the word "Download" on the right hand side of the guidelines document found at the top of this page
When the File Download Dialogue Box Appears
2. Click “Open”
When the document opens
3. Read the document carefully and type your answers onto each form
NOTE: The two forms to be filled out by clubs are found on page 10 and 11 of the guidelines document
4. Once the forms are filled out in detail print the entire document
NOTE: PDF's cannot be saved, only printed, unless if you have Adobe Acrobat Professional
5. Sign all forms
6. Turn in, or Mail documents to the appropriate contact.
Remember Rotary Clubs, in addition to the club forms, you need to make sure that all student campers, your club is sponsoring, have filled out all necessary forms and turned them in.

ROTARY CLUBS MUST HAVE THE APPLICATION PROCESS, INCLUDING INTERVIEWS COMPLETE AND FINAL DECISIONS MADE WITH APPLICATIONS RETURNED TO THE RYLA COMMITTEE BY:
